Transcript Requirements

Necessary Information for Academic Transcript Request

1. The name under which you attended and any/all name changes since leaving Carthage.
2. The approximate date of last attendance or graduation date.
3. Your current address and daytime telephone number.
4. A specific address where you need the transcript(s) sent, including an office or person, if possible.
5. Your signature giving us permission to release the transcript(s).
