The Modern Language Scholarship Application

Your scholarship application has been successfully submitted. However, the recommendation portion of the application must be submitted for your Modern Language Scholarship application to be complete. This is required for French and German Scholarship applicants. (For Chinese and Japanese, if you have not formally studied the language in school, the teacher recommendation can be omitted.)

The written recommendation from your high school language teacher must be postmarked or submitted online by December 3, 2011.

Your teacher may submit this recommendation by submitting the form below (scroll down), by email to admissions@carthage.edu, or by fax to 262-551-5762. (For convenience, your teacher can use www.carthage.edu/modernlanguage to access this form.)

The recommendation must include:

  • Your name and high school.
  • Your teacher's name, high school address, high school phone number, and email address (if applicable).
  • Your teacher's comments on all parts below:
