The Modern Language Scholarship Application

Your scholarship application has been successfully submitted. However, the recommendation portion of the application must be submitted for your Modern Language Scholarship application to be complete. This is required for French and German Scholarship applicants. (For Chinese and Japanese, if you have not formally studied the language in school, the teacher recommendation can be omitted.)

The written recommendation from your high school language teacher must be received at the Carthage Office of Admissions by December 11, 2009.

Your teacher may submit this recommendation by e-mail to admissions@carthage.edu, by fax to 262-551-5762, or through this form. (For convenience, your teacher can use www.carthage.edu/fgrec to access this form.)

The recommendation must include:

  • Your name and high school.
  • Your teacher's name, high school address, high school phone number, and e-mail address (if applicable).
