Student-Athlete Participation Forms


Athletic Participation Forms Procedure
To avoid being withheld from participation, all Carthage College student-athletes must follow these instructions exactly as stated. Click on each link for the form.

Do not send any of the following forms in the mail. All forms are to be hand delivered to the athletic training staff during your athletic team check-in. DO NOT MAIL — DO NOT GIVE TO COACHES.

Sports Pre-participation Physical

Carthage will not provide on-site physical exams. Our team physician Dr. Bryant Walrod is available by appointment if needed. Call (262) 656-3525 to arrange.
• No valid physical = no practice.
• Administer after June 1, 2011.
• Performed by a medical doctor (MD) or doctor of osteopathy (DO) only.
• Physician's name, signature, address and telephone number are required.

Medical History

• All athletes including returners must complete.
• Complete all questions; explain any YES answer in space provided.
• Read and sign back page (two signatures required).

Insurance Emergency Information

• Complete all sections legibly.
• Provide all numbers requested.
• Parent (policy holder) signature

NEW—ATHLETES MUST HAVE PRIMARY INSURANCE COVERAGE PRIOR TO PARTICIPATION

The policy must:
• Cover intercollegiate athletics
• Be applicable to coverage in Wisconsin
• Be retained for the length of athletic participation (including pre-season and nontraditional seasons)

In addition to the "2011-2012 Athlete Insurance and Emergency Information" form from above, the athlete must provide a copy of a current insurance card.

EIIA Individual Athletic Accident Insurance Plan

For those student-athletes without primary insurance coverage, EIIA offers a plan for $600 per year. This charge is applied to the student-athlete's Carthage business office account. Contact Laurie Jensen, Head Athletic Trainer (ljensen@carthage.edu), 14 days prior to the start of your season if you are interested in purchasing this policy.

ADHD Medical Exception

Any athlete prescribed ADHD medication must read guidelines carefully.
• Annual follow-up documentation required

Athletic Training Room Policies and Procedures


All student-athletes must abide by the policies and procedures of the athletic training room. If an athlete fails to do this, he or she will be asked to leave the athletic training room. Download the Athletic Training Room Policies and Procedures.
